I ask because Celebrity are apparently charging £350 extra for a fly/cruise package out of Civitavecchia next summer; or, to put it the other way round, if you buy a 'cruise-only' deal and arrange your own UK - Civitavecchia transport, you can save £350 per person....
Then your flights.. way less than £350!

As for a direct route to Civitavecchia from Fiumicino, you may have to catch a taxi (which would cost a fortune) or enquire whether they have a bus available. The other option is to hire a minibus or limo together with a few other people. My last time at Fiumicino was in 2002.

Italian trains are usually reliable - the only thing which happens from time to time is a strike.

A slight correction: Ostia is on the coast (as Lido di Ostia) and there is no service from Fiumicino over the Tiber to there. I think you meant the Ostiense station, which is just after Trastevere on the way to Termini. Close by the Ostiense station at the Piramide (Pyramid) a train can be caught for Lido di Ostia on the Roma-Lido line.
The service that stops at Trastevere and Ostiense from Fiumicino, as Pam pointed out, is a metropolitan service – the Leonardo Express is Fiumicino-Roma Termini and vice-versa. This metro service, from memory, does not go to Roma Termini, but Roma Tiburtina which is way off track. The metro service is mostly used by commuters, it takes longer (by approx. 10 minutes), it is cheaper, it may not be as comfortable and it may not have as much room for luggage. Of the two stations mentioned, Trastevere is the closest to Civitavecchia, it will shorten the trip there by only a few kilometres.

Tom, this is your best and most speedy option. The price does not seem to be too bad and about the norm for that distance. I have used taxis twice from outer Rome (suburb adjacent to the ring road/freeway interchange for Naples), and paid both times €45 (once L90,000). Civitavecchia is about twice that distance (maybe just short of it). Visit this site and compare your price: Rome Airport Shuttle.
