I didn't read it all yet, but by the summary, looks like methods of Bureau Veritas didn't allow to sufficiently detect and prevent the accident and before all NCL' negligence and lack of adequate maintenance were responsible of the drama.
Sad.

Yes, the summary says that the accident was caused by negligence of NCL and Bureau Veritas.I wonder to which extent certain individuals will be held responsible. (e.g. crew members)
